Arrival Time

Please arrive 15 minutes early to allow time to complete paperwork in the office OR complete the print out the forms associated with your practitioner and arrive 5 minutes early to your scheduled appointment.

Financial

Please feel free to call or email our office with any questions about insurance coverage and we will be happy to help. It is our aim to help you recover the most from your health care benefits. You may also want to call member services provided on the back of your insurance card to determine coverage benefits.

Please remember that it is ultimately your responsibility to know your benefits and we are not responsible for incorrect or incomplete information given to us by your insurance provider.

 Cancellation

If you are unable to keep an appointment, we ask that you kindly provide us a 24 hour notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
